Plumbing Service Contract Template
Plumbing work often involves concealed pipes and uncertain access. A clear service agreement helps distinguish the diagnostic visit, approved repairs and any later change order.
Important: this is not an executed contract
This educational starting point is not a jurisdiction-approved form. Requirements for licensing, deposits, notices, cancellation, consumer protection, and signatures vary. Have a qualified professional review the final document. Saving or downloading a draft does not sign it, initiate payment or change an invoice.
Separate diagnosis from repair authorization
State whether the initial visit covers inspection only, a defined repair or both. Record the service call fee and any hourly minimum. For leaks behind a wall or below a slab, avoid implying that concealed damage is included in a price for opening and inspecting the area.
Identify fixtures, materials and restoration
List the sink, toilet, valve, water heater, pipe section or drain being serviced, relevant model or material specifications, and included pressure or leak testing. Explain who will patch walls, replace tile, dry affected areas and restore landscaping. Use a separately approved change order for additional pipe replacement or remediation.
Document access and service interruption
Confirm water shutoffs, tenant or homeowner access, safe work areas and planned interruptions. Specify inspection or permit responsibilities when required. If the repair depends on further investigation, describe the next decision point and cost authorization rather than asserting a guaranteed outcome.

Frequently asked questions
Does a service agreement cover every hidden pipe problem?
No. Identify known conditions and describe how new findings will be documented and separately approved.
Should drywall repair be included?
Only if the contractor is actually agreeing to perform it. Spell out access holes, patching, painting and other restoration.
Can an estimate become the agreement?
An accepted estimate may form part of an agreement, but applicable rules may require additional terms, notices and signatures. Review before use.
